There are three ways you can use this form to apply for the 33^rd Annual Northeast Regional Invitational Tournament sponsored by the Greenfield High School Hockey Parents Association.

2) You can select Download Application Form - WORD Doc File (to deal with the form as a WORD document). You can then print it on your computer and fill it in. Or 3) If you have Adobe Acrobat Reader already on your computer, and you'd rather print the form as a PDF file then select Download Application Form - PDF File, print the form and fill in.

Please make check payable to GHS Hockey Parents Association and send to:

$150.00 deposit required (or full tournament fee of $600.00) with this application. The full amount is due 30 days prior to the start of the tournament. A copy of the team’s USA Hockey registration roster must be received by this date as well.
