33^rd Northeast Regional Invitational Tournament

TEAM ROSTER FORM

March 7 - 9, 2008

There are two ways you can fill in the blank roster form as part of the application process for the 33^rd Annual Northeast Regional Tournament sponsored by the Greenfield High School Hockey Parents Association. 

1)   You can select Download Roster - WORD Doc File (to deal with the form as a WORD document). You can then print it on your computer and fill it in.

2)  If you have Adobe Acrobat Reader already on your computer,  and you'd rather print the form as a PDF file then select Download Roster - PDF File, print the form and fill it in.

The completed roster table shown below is there as an example, as a guide. Note that Home|Away stands for home and away jersey numbers. List players in ascending numerical order based on home jersey numbers. If home and away numbers are different, list both numbers using the vertical bar symbol," |  “,to separate the numbers. This table will appear exactly as I receive it in the tournament program, so please verify spelling and jersey numbers. Feel free to use first name nicknames if you so desire. All players listed in the table must also be on the team’s USA hockey registration roster. Please send the completed form to:
